No choice, no. There are gipsies and travellers that don't pay housing tax or something like that, but otherwise every time you buy a packet of chewing gum you're paying 19-24% in tax. But taxes don't just cover sex changes. It covers things that will save your life, roads, free education (until college), etc.

You can't walk into a clinic and order a vagina or penis. There's shrinks to determine whether you're actually 'suffering.' That includes dressing up and presenting yourself as what you intend to be for at least half a year while starting on hormones. Given the limited amount of cases each year it doesn't make much of a dent in the budget. Hell, if you're a woman and capable of convincing a shrink you're suffering cause of your small tits they'll pay for implants, course that's not a bad thing.
And you probably still have to pay money for medication and maybe hospital stay time and things like that. Here in Ireland it costs me $8 per month of medication. It used to be free.
As Haki says, you can't just walk into a surgery and demand a sex change. There are no doubt wait lists, criteria checks, etc.

In the UK you actually have to jump through a lot of hoops to get SRS surgery. And on some level that's definitely fair, it shouldn't be a decision one makes lightly, but I think it's wrong to misrepresent it as a "crazy" thing to do. And I'm not slagging off anyone who doesn't support it being publicly funded, but I completely do.
