What I can gather: ~somewhat redundantly named "Space Galaxy" ~Two cascading reverbs with a master (graphics suggesting long decay -> short decay) ~Depth, Reverb, and notched Decay Tone ~Possibly a pre-verb fuzz ~sicknasty paint job
Given Death By Audios usual size choices and the amount of stuff in it, the verbs are probably digital.
for what it's worth, this video is from what, like two years ago, he says "you woun't be able to buy these" but a rotary decay tone is pretty coo.
