My guess, just a small hinge would be the simplest way... then just a pot with a gear going to it like in a normal wah, however going as far down as a hinge might, Terra Firma may be an issue, adding a spring to the end would be good but removes the option for cocked wah sounds... this is how i'd do it if I was going to but it's still not simple http://www.diyguitarist.com/DIYStompboxes/DIY-Wah2.htm
It's a 1590B enclosure with a 1590A bottom panel for those interested... and has been discussed with Blurz, the only design point that makes it difficult is the gear for the pot.

Tech21 made a really small wah called the Killer Wail.
