Bands that think they need multiple full stacks of things like Sunn heads and Orange Thunderverbs, but they really can't play them or bring the bruutz.
Alternatively, bands that think that a Marshall MG100 head is an acceptable thing to play in public.

Yet another doom metal band that just plays sabbath riffs, tuned down an octave or so, and slowed down to about 30 bpm.

"Americana"
imitating Radiohead
fourth level of derivation blues-rock (i.e. people ripping off people who ripped off people who ripped off people who ripped off the original artists)
jam bands that aren't Phish
fake grunge
fake punk e.g. Green Day
neo-hippy singer-songwriters who can't write e.g. Sufjan Stevens
emo
rap with lame backing tracks that overuse one or two samples
New Country female vocalists who sound like a crap version of Bonnie Bramlett

Bands that say they are metal and are really just a crust band.
Extremely fast bands that are tuned so low you can't hear anything.
Bringing full stacks to basement shows.
