I'm guessing from the way it says "Fuzz Hugger 1134" that it's a Fuzz Hugger 1134 with a custom paint job no?
FuzzHugger (fx) 1134 Fuzz - Distortion - pedal - the Hell Fuzz. video
A fiery shape-shifting, multi-textured pedal! Full on, it's a blistering, splattery synth fuzz explosion...back off the gain trim and you'll find several distortions and growly fuzz textures--and even get into some nice overdrives! Very versatile, with top-notch tones all along its range! There's nothing else like it! --Tom FuzzHugger.com

Yep, the 1134 fuzz was an earlier Fuzzhugger offering that eventually got phased out, but led the way to other current Fuzzhugger pedals. I believe that Tom has said that The Chalice (now a 2010 limited offering) can get the same tones as the 1134, and the Chalice is essentially a non-oscillating Ab-Synth, so there are (at least) two current pedals in the FH(fx) lineup related to the 1134.
