Help fix my preamp

...in my bass. It's an Ibanez TR Expressionist. When I got it the active position didn't work. The pre-amps in the TR's were known to be faulty. I removed the op-amp, put it back in, checked everything else, changed the battery, etc and it started working again. Well eventually it stopped for a day or two, then started working again on its own
. Now it's broken again, has been a for a few weeks, & I need it fixed. When I flip the switch to active it simply cuts the signal.
I was thinking, start with swapping the opamp (tl062cp), then on to the caps and trani's. Any ideas? The bass needs a good set-p anyway, I may just take it in and kill 2 birds with one stone.
