I can tell you that it is an Accutronics tank with a Hammond 124-000166 reverb circuit board attached. I modified the board with addition of of a dual-ganged 100K mix control, a Fender-style dwell control, and I moved the 50K reverb output volume trim pot control out to the front panel with a full size potentiometer. I also had to wire up input and output jacks and hook up power. The Hammond 124-000166 circuit board is, I believe, found in Hammond Concorde Series organs from the 70s and 80s.
The case was originally the base for an early seventies German-made Dual record player.

The one I got from Tomkat Pedals has become an always on kind of thing for me, such a fun tone to mess with. Can not recommend getting one enough.

If y'all contact Haberdasher on the madbeanpedals forum he can etch you a board.
Be warned though, even though it uses a SMPS rather than a power transformer it can still give you a nasty shock... as I discovered yesterday when testing the power switch forgetting that I had left the transformer center tap lead poking out the top of the enclosure
