I actually went with a different approach when I did this for a guy I know who was complaining about a Polish place he went to in Chicago playing nothing but Roxette his entire stay:

My amp has to have the treble turned down, because it has a naturally high response on the upper end, so I'd probably turn the highs up, roll the bass down, crank the volume, and then do the same on my tube driver. I will destroy your ear drums, fucker.
Edit: This with an ebow. I forgot that I had one laying around, and those fuckers will create the most annoying high end on a note, EVER.

I want to list the Megalith on here as painful. I like the tone, don't get me wrong, but the amount of volume on tap is just absurd. It hurts so bad, I can't even use it!! lol
Also, most Dwarfcraft pedals. I LOVE Dwarfcraft, but good God there has got to be a way to simmer that shit down a bit.

TheSyle fuzzes I've had have all been able to make some pretty ugly noises when cranked. The Singularity being the most ridiculous with the option to treble boost and filter the post and pre gain EQs.
What about doing the opposite in the OP situation. Instead of making a really painful noise, you just made your guitar sound like utter shit. Sometimes I enjoy making the ugliest dying death tone just to see what notes pop out of the mix of fuzz and slush. I've always wanted to play a show with a dying tone the entire time.

I always found the Guitar bleeps in Rapeman's Hated Chinese to be kinda painful.
Now after playing with the Geiger Couter for a few days you can get pretty painful with it.
I think my all time fav is the reverberated clang that Rowland S. Howard violently attacked eardrums with.

I would make a 1 second loop of Treble boost+Whammy 2 octaves up+ plus an ugly fuzz and play this chord and just let it repeat over and over while I mess with my amp's eq:
E---0
B---4
G---7
Or do it with my POD HD straight into the board and have our soundguy turn the PA up until ears bled and brains melted.

I think that as far as construction goes a sovtek muff could probably be used much like a rock or brick to hit people with while taking the least damage to the actual pedal.
