Something like this is a good start:

Pretend the send/return a/b in the diagram are actually the two stompswitches. Use a traditional true-bypass wiring diagram for those. Then order switcher toggle > output jack.
You'll need to rewire the LEDs as well, so that the third pole of each stomp turns on/off their respective red LED. Wire up the yellow LED so that it's always on when a power supply is connected.
It might be easier to start with two brand new stomps, but if you're ginger with your iron you could reuse the existing ones.
